We ask that you fill in as many details as possible about yourself and your pet as possible.

Once Dr Cathy and the team have received and reviewed your information, we will be in touch via phone or email.

Your phone number assists with follow up phone calls as well as getting in touch on the day or to confirm your appointment.

Please let us know the name of your pet (for example, Samuel).

Please let us know the sex of your pet (for example, male or female).

Please let us know the age of your pet (for 5 years 3 months old).

Please tell us the type and breed of your pet (for example, Rabbit, Cat, Dog (King Charles Cavalier Spaniel).

Please provide as much information as possible. This may include current concerns or situation, existing conditions or incidents, service type (i.e. vaccinations or anxiety) - as well as any additional information you feel is relevant.

9:00am - 5:00pm
Monday, Tuesday, Wednesday, Friday.  
Saturday mornings by prior arrangement.
